H: HEENT:  head-no trauma, eyes-no jaundice, clear anterior chambers, no obvious catarracts, conjugate gaze, ears-canals clear, tms intact, pearly gray,nose-patent,intact, no mucosal lesions, throat-intraoral exam ok, no mucosal lesions, throat normal. 
Neck: Neck: supple, carotids full and no bruits, no thyromegaly, no lymphadenopathy present, normal range of motion 
C: Chest:  no respiratory distress noted, respirations physiologic rate, breath sounds equal in 4 quadrants of posterior chest, there are no rales, wheezes, or rhonchi present 
Heart: Heart regular rate and rhythm, no rubs, murmurs, or gallops noted. 
Abdo: Abdomen:  soft, nontender, no mass, no hernia, no guarding, no rebound tenderness, no ascites, non obese 
B: BJE: no clubbing, no cyanosis, no edema, pulses full in all extremities, normal muscle tone, no varicosities, neg Homan's sign, no obvious weakness, normal gait 
Neuro: Neuro: Alert and oriented without obvious deficits. CN 2-12 are intact.Motor is 5/5 bilaterally. Gait is steady. Romberg's and pronator drift are negative. DTR's are  2+ and equal bilaterally.Coordination is intact. 
P: PLAN: 
B: Breast: breasts are symmetrical, no mass is palpable, no dimpling, orange pealing, or nipple discharge is detectable, no redness or induration is present. 
R: RECTAL: normal external appearance, normal tone, no masses palpable, hemoccult negative. 
Pro: Prostate smooth, non-enlarged, no nodules, and non-tender. 
Shou: Shoulder movements full, with painless full abduction.
elb: Elbows straight, with no synovial tenderness.
hands: No synovitis present in small hand joints, and flexor tendons with good grip strength.
